Mercer Human Resource Consulting, in association with Sun Life Assurance Company of Canada, is pleased to provide personal benefits insurance program geared specifically to the needs of Advocis members. From Life and Critical Illness Insurance to Accidental Death and Dismemberment, Extended Health and Dental Care Insurance coverage, the program provides comprehensive benefits in a seamless and easy-to-buy package.
 

Who is eligible?

 
This program is open to all Advocis members between the ages of 18 and 65, who meet the following conditions:
  • The member must be actively working a minimum of 30 hours per week;

  • The member must select a minimum of $50,000 for Life Insurance and/or Critical Illness Insurance to be eligible for the other plan benefits, and

  • Medical evidence is required and subject to approval by the underwriter, Sun Life Assurance Company of Canada.

 

Coverage is also available for Spouse and Dependent Children of Advocis members.
  • Spouse is defined as a legally married spouse or person of the opposite sex or same sex, with whom the Insured has been cohabitating for at least one year and who is publicly represented as the Insured member's spouse. Only one person at a time can be covered as a Spouse under the plan.
     
Dependent Child is defined as:
  • Unmarried dependent children, including adopted children and stepchildren, of the member or the member's spouse who are under the age of 21 (age 25 if dependent is full time student and entirely dependent on the member for support - age 26 for the province of Quebec), and

  • Unmarried dependent children of any age who are incapable of self-support because of physical or mental disability.

As mentioned, the insurance program has been designed to provide Advocis members and their families with access to excellent coverage in a cost-effective and seamless manner. Once you've selected one of the "core" coverages of Life and/or Critical Illness Insurance, you can add Accidental Death and Dismemberment, Extended Health and Dental Care Insurance.

Life Insurance

 

Life Insurance is one of the most important things you can leave your family. By having adequate coverage, you get the piece of mind that comes with knowing your family is financially protected.

  • A minimum of $50,000 (except if you are age 50 or over, where the minimum is $30,000) to a maximum of $500,000 is available.

  • Coverage is purchased in increments of $10,000.

  • Coverage is available for spouse and dependent children.

  • Coverage can be purchased for a member's spouse (however the amount cannot exceed the amount purchased for the member).

  • If the Insured member's/spouse's life coverage terminates for any reason other than their request, the Insured may convert up to $200,000 of coverage to an individual life policy without evidence of good health. If coverage terminates due to the termination of the plan, the Insured must have at least 5 years prior coverage under the plan to convert to an individual policy.

  • The Waiver of Premium provision is included if a member/spouse becomes totally disabled before the age of 65 for a continuous period of 180 days. Premiums will be waived until the Insured's 65th birthday, or until the Insured is no longer disabled.

    • Total Disability is defined as the Insured is unable to perform duties of any occupation he/she is or may become reasonably qualified for by education, training or experience and is not working elsewhere.

  • No benefits will be paid if the member/spouse commits suicide in the first 2 years of coverage.

  • Dependent Child Life:

    • A $10,000 benefit is available per dependent child. A $1,000 benefit is available for each child less than 15 days old.

    • Medical evidence is required for your child/children at the time of enrollment. Any children born subsequently will be covered automatically provided you already have family coverage under the plan.

    • If both parents are members of Advocis, the dependent child(ren) may only be covered by one member.

    • Premiums will be waived if the member qualifies for waiver of premium benefits under the Life coverage.

    • No benefits will be paid on behalf of any dependent child who commits suicide in the first two years of coverage.

Critical Illness Insurance

 

One is rarely prepared for the emotional and financial impact of a critical illness. If it does happen, the potentially life-changing consequences that could accompany a serious illness, are possibilities most of us would rather not face. The associated costs could jeopardize your standard of living and affect your quality of life. Dealing with the impact of a major illness is difficult enough. This plan helps provide financial peace of mind so you can concentrate fully on the thing that matters the most - your recovery.

  • The plan helps provide a LUMP SUM payment when the insured member has survived 30 days (90 days for cancer) after the initial diagnosis of one of the 10 covered "Insured Critical Illness Conditions".

  • A minimum of $50,000 to a maximum of $300,000 is available.

  • Coverage is purchased in increments of $10,000.

  • Coverage can be purchased for a member's spouse (however, the amount cannot exceed the amount purchased for the member).

  • The benefit is payable on the first Insured Condition only; coverage terminates with this payment.

  • The Insured can choose how to use the payment and the benefit is paid whether or not the Insured recovers.

  • The benefit is subject to a survival period of 30 days unless otherwise specified in the Insured Critical Illness Conditions.

  • Insured Critical Illness Conditions:

    • Heart Attack (Myocardial Infarction) - death of a portion of heart muscle resulting from a blockage of one or more coronary arteries. Diagnosis based on both:

      • New ECG changes, which support the diagnosis of a heart attack, and

      • Elevation of cardiac (heart) enzymes.

    • Coronary Artery Bypass Surgery - heart surgery to correct narrowing or blockage of one or more coronary arteries with bypass grafts, but will exclude:

      • Non-surgical techniques, such as balloon angioplasty;

      • Laser relief of an obstruction; and

      • Other non-bypass techniques.

    • Stroke - diagnosis of a cerebrovascular incident causing infarction of brain tissue due to haemorrhage, thrombosis, or embolism, producing measurable neurological deficit persisting for at least 30 days following occurrence of stroke. Specifically excluded from this coverage is transient ischemic attacks (TIA).

    • Cancer (Life Threatening) - malignant tumour characterized by uncontrolled growth and spread of malignant cells and invasion of tissue. Excluded cancers are:

      • Stage A prostate cancer;

      • Pre-malignant lesions, benign tumours or polyps;

      • Non-invasive carcinoma in situ;

      • Any skin cancer, except invasive malignant melanoma into the dermis or deeper, and

      • Any tumour in presence of human immunodeficiency virus.

No coverage if symptom or medical problem leading to cancer diagnosis commenced within 90 days following first premium due date.

  • Kidney Failure - irreversible failure of kidneys from any cause, which necessitates either regular treatment by dialysis or kidney transplant.

  • Paralysis - complete loss of use of two or more limbs through paralysis that has lasted for a continuous period of 180 days and which is deemed to be permanent.

  • Major Organ Transplant - surgery, as the recipient, for transplantation of any of the following organs or tissues: heart, liver, lung, kidney, or bone marrow.

  • Multiple Sclerosis - diagnosis of multiple sclerosis producing at least two episodes of well defined moderate neurological abnormalities, which have persisted for a continuous period of 180 days and confirmed by modern imaging techniques.

  • Blindness - permanent and uncorrectable loss of sight in both eyes. The corrected visual acuity must be either worse than 20/200 in both eyes, or the field of vision must be less than 20 degrees in both eyes.

  • Deafness - permanent loss of hearing in both ears, with an auditory threshold of more than 90 decibels.

Accidental Death and Dismemberment Insurance (AD&D)

 

Most of us will never experience a serious accident, but for those who do, the consequences could bring loss of life or limb. That in turn could pose a serious threat to the financial stability of your family. Accidental Death and Dismemberment Insurance (AD&D) is designed to provide additional financial protection to you and your family in the event of an accident that results in death or serious injury.

  • A minimum of $50,000 to a maximum of $500,000 is available.

  • Coverage must be purchased in units of $10,000.

  • Coverage is not subject to medical underwriting.

  • The amount of AD&D coverage is limited to the greater of $200,000 or two times the amount of Life coverage in force (subject to the overall plan maximum).

  • 24-hour worldwide coverage. AD&D protection is provided 24 hours a day, 365 days a year, anywhere in the world.

  • Single and Family coverage is available:

 

Spouse and Dependent Child(ren)

Spouse covered at 40% of member benefit - Each Child covered at 10% of member benefit.

Spouse Only

Spouse covered at 50% of member benefit.

Dependent Children Only

Each child covered at 20% of member benefit up to a maximum of $50,000.

 

Benefits available at no additional cost include:

 

Repatriation Benefit

In the case of accidental death that occurs more than 100 kilometers from home, the AD&D Plan may reimburse for expenses for returning the body. Maximum amount payable is $10,000.

 

Family Transportaion Benefit

If a covered person suffers an accidental loss and is hospitalized more than 150 kilometers from home, the AD&D Plan may reimburse accommodation and travel expenses for a member of the immediate family to be present. Maximum amount payable is $5,000.

 

Spouse Occupation Training Benefit

In the event of the Insured's accidental death, the Insured's spouse may be reimbursed for training expenses, if necessary, to become qualified for employment in a new occupation. For an approved training program the maximum benefit is $5,000.

Extended Health Care Insurance

 

Extended Health Care Insurance reimburses the Insured for eligible health care expenses that are not covered by provincial health plans. There are two plans to choose from; a Basic Extended Health Care Plan, with no prescription drug coverage or an Enhanced Health Care Plan which consists of all the details listed below:

  • Medical evidence is required for the member, spouse and dependent children. Any children born subsequently will be covered automatically provided the Insured already has family coverage under the plan.

  • Available on a single, couple or family basis.

  • No deductible applies.

  • Hospital benefit - 100% of the following expenses for hospital care are reimbursed if incurred in your province of residence:

    • Semi-private hospital accommodation.

    • Out-patient services in a hospital.

    • Convalescent hospital accommodation expenses to a $20-per-day maximum for up to 180 days per illness. If ordered by a doctor for rehabilitative purposes, after at least five days hospital confinement and starting within 14 days of release from hospital.

  • All eligible major medical expenses are reimbursed at 80% of reasonable and customary charges for benefits provided.

  • As an added convenience, Insured members can access their benefit/claims information through the Internet at Sun Life Financial's web site.

  • Highlights of Covered Benefits:

    • Prescription drugs - pay-direct card in use, which means no forms for you to complete -- $3,000 annual maximum per person and a $5 dispensing fee cap.

    • Ground ambulance and emergency air ambulance.

    • Paramedical practitioners -- $500 per practitioner per person per year (includes massage therapy, chiropractor, physiotherapy, speech therapy, chiropody, podiatry, naturopathy, osteopathy, psychology).

    • Custom made orthotic inserts are covered to a maximum of $350 per year and custom made orthopedic shoes are covered up to $500 per year when prescribed by a doctor, podiatrist or chiropodist.

    • Accidental dental -- $2,000 maximum per person per year.

    • Private Nursing Duty -- $25,000 maximum per person every three calendar years.

    • Hearing Aids -- $500 maximum per person per lifetime.

    • Services of an ophthalmologist or licensed optometrist -- $50 maximum per person every 2 calendar years.

    • Emergency Travel Coverage (max. coverage 60 days) 100% reimbursement for emergency hospital and medical expenses while traveling outside the Insured's province of residence. Subject to a lifetime maximum of $200,000 per person.

Dental Care Insurance

 

Routine trips to the dentist and unexpected dental costs can add up. Dental Care Insurance reimburses the Insured and eligible dependents for covered dental expenses.  

  • The member must select Extended Health Care in order to be eligible for Dental Care Benefits.

  • Medical evidence is not required.

  • Available on a single, couple or family basis.

  • Subject to a maximum of $500 per person during the first plan year and a maximum of $1,500 per year thereafter.

  • No deductible applies.

  • All eligible expenses are reimbursed at 80% of the current fee guide for General Practitioners in the Insured's province of residence.

  • As an added convenience, Insured members can access their benefit/claims information through the Internet at Sun Life Financial's web site.

 

Highlights of Covered Benefits:

  • Basic Service includes:

    • Routine examinations and diagnosis including recall examinations every 9 months.

    • Dental x-rays and interpretations, including full mouth x-rays once every 2 plan years and bitewing x-rays once every 9 months.

    • Routine diagnostic and laboratory procedures.

    • Light scaling, 1 unit every 9 months.

    • Polishing of teeth, 1 unit every 9 months.

    • Topical fluoride treatment, once every 9 months.

    • Oral hygiene instructions, lifetime limit once per person.

    • Space maintainers for dependent children under 19 years of age.

    • Fillings and retentive pins.

    • Pit and fissure sealants for dependant children under 19 years of age.

    • Extractions.

    • Local or general anaesthesia.

    • Endodontic Services (root canal therapy, root canal fillings and treatment of disease of the pulp tissue).

    • Periodontic Services (treatment of bone and gum disease, including scaling and root planning to a maximum of 8 units per plan year).

    • Surgical Services related to the gums and the bone surrounding the teeth.

    • Repairs or relining and re-basing of existing dentures.
